What is a (CES)?
The Salt Lake Valley Coalition to End Homelessness (SLVCEH) Coordinated Entry System (CES) operates throughout Salt Lake County. It streamlines and simplifies the process of connecting people experiencing a housing crisis with housing programs throughout the community.

How Does it Work?

The Coordinated Entry team collects referrals from community access points to create and maintain a By Name List (BNL). This shared community resource helps connect individuals and households experiencing homelessness or a housing crisis in Salt Lake County with available housing opportunities. Households are added to the BNL through referrals from community partners, including shelters, outreach teams, behavioral health providers, and others.
Each week, community partners work together to review housing opportunities and prioritize households based on HAST (Housing Assessment Screening Tool) scores, program eligibility, and level of need, helping ensure those facing the greatest barriers are connected to housing support first.

Who is Eligible?
Anyone experiencing a housing crisis is eligible to be referred to the the Coordinated Entry system. Current programs being coordinated serve a variety of demographics including:
Youth ages 18-25
Veterans
Families with children ( includes 3rd trimester pregnancy)
Chronically homeless adults (12+ months of homelessness in the last 3 years)

What housing options are there?
Any housing program can operate through Coordinated Entry. We are currently prioritizing households in need for the following:
- Family Rapid Re-Housing: families with children
- Youth Rapid Re-Housing: transition-aged youth, ages 18-25
- Youth Permanent Supportive Housing: transition-aged youth, ages 18-25
- FYI vouchers: transition-aged foster youth, ages 18-25
- VASH vouchers: Veterans
- Bridge vouchers: chronically homeless adults and families with children
- Bud Bailey: chronically homeless families with children
- Palmer Court: chronically homeless adults and families with children
- Grace Mary Manor: chronically homeless adults
- Wendell: chronically homeless adults
- Magnolia: chronically homeless adults
- Kelly Benson: chronically homeless adults age 55+
How do I get help?
If you are experiencing homelessness and are receiving help from a professional homeless services agency, ask them to connect you to Coordinated Entry.
Or, you can meet with the Coordinated Entry Navigator to be added to the By Name List. The CE Navigator has weekly office hours at the following locations around Salt Lake County, and can help you connect to housing programs:
